The 2009 federal and state K-1, Member Participation Report for accident year 2009 and blank 2009 Insurer’s Report were mailed to each member on April 13, 2010.  A copy of each will be posted to the secured section in the member’s area of the website.  Please return the completed and signed 2009 Insurer’s Report with the Indiana State Page 14 by July 1, 2010.
